Job summary

Gardens Healthcare LLC is seeking an MDS Nurse to complete MDS assessments for residents across HMO, Medi-Cal, and private pay. The role supports the interdisciplinary care plan, coordinates assessment dates, and leads care conferences with the IDT to ensure comprehensive resident care.

You will be responsible for quarterly assessments, updates to care plans, and providing direct patient care when needed, under the Director of Nursing Services.

Responsibilities

• Completes MDS Assessments for HMO, Medi-Cal, and private pay residents.
• Initiates the interdisciplinary care plan according to the RAP summary and orders.
• Completes quarterly assessments and updates care plans.
• Coordinates MDS Assessment Reference Dates with IDT and maintains logs.
• Leads Care Conference meetings and ensures full discussion by the Interdisciplinary Team.
• Provides direct patient care as needed per Director of Nursing Services.

REPORTING: The MDS Nurse reports to the Director of Nursing Services.JOB DUTIES:1. Completes MDS Assessments for HMO, Medi-Cal, and private pay residents according to OBRA Regulations.2. Identifies the triggers and completes the R.A.P. summary.3. Initiates the residents interdisciplinary care plan according to the R.A.P. summary, current diagnosis and physician orders.4. Completes quarterly assessments.5. Assists in the coordination of plan of care by following up on needs identified related to the assessment.6. Reviews and updates care plans quarterly.7. Completes Discharge Tracking for HMO, Medi-Cal and private pay residents according to OBRA Regulations.8. Coordinates MDS Assessment Reference Dates with IDT Members and maintains log as directed by Director of Nursing Services or Administrator.9. Ensures appropriate scheduling of Care Conferences and communicates the schedule to IDT Members at least 2 weeks prior to meeting.10. Leads the Care Conference meeting and ensures Residents are fully discussed as an Interdisciplinary Team.11. Participates as an Interdisciplinary Team member as directed by the Director of Nursing Services.12. Complies with Facility Polices and Procedures.13. Attends in-service meeting as directed.14. Serves as a role model for nursing staff.15. Promotes a positive, calm and effective work environment.16. Will provide direct patient care as needed and as directed by the Director of Nursing Services17. Serves on committees as assigned by the Director of Nursing Services.18. Works hours approved by the Director of Nursing Services and/or designee.19. Other tasks as assigned by the Director of Nursing Services or Administrator.REQUIREMENTS:1. Graduate of an accredited school of Nursing2. Possesses a valid and current RN license3. 1-2 years experience in long term care preferred4. Possesses effective written and verbal communication skills5. Good physical and mental health6. Must be able to perform any physical requirement of the position which includes lifting dead weight or unassisted weight of 40 pounds.7. Is capable of following oral and written instructions8. Has training, knowledge and experience which allows for general supervision of responsibilities9. Is capable of working independently10.
