Qualifications

• Requires nursing education background (Bachelor's or Diploma/Associate in Nursing).
• Active Florida RN license.
• CCM certification preferred.
• Strong communication with physicians, patients, families and staff.

Responsibilities

• Assess patient and family scenarios for clinical appropriateness and care setting.
• Plan ongoing continuum of care and establish management strategies.
• Collaborate with the care team to optimize quality of care, utilization, cost and patient satisfaction.
• Reassess progress and adjust care plans as patient needs evolve.
