• Provides patients with assistance in performing activities of daily living.
• Provides equipment/supplies for and assists patients with oral and personal hygiene.
• Prepares patients for meals/snacks/liquids and assists with feeding; assists in assessments.
• Measures and records TPR, blood pressure, height, weight, intake/output, and bowel movements.
• Collects specimens (urine, feces, sputum) as required.
• Reports changes in patient condition to the RN; performs selected diagnostic tests under supervision.
• Applies simple dressings; discontinues IV peripheral therapies; performs EKG.
• Obtains accuchecks and informs nurse; assists with Foley catheter insertion and removal.
• Cleanses pressure injuries; positions and transports patients safely.
• Responds to call bells and emergencies; ensures patient safety.
• Adheres to safety protocols, monitors for unsafe acts, and participates in drills.
• Maintains competence through in-service education.

Job Description

Performs selected diagnostic tests and prescribed treatments. Provides patients with assistance in performing the activities of daily living. Assists health care team in ensuring patients safety.

Job Responsibility

1. Provides patients with assistance in performing the activities of daily living
2. Provides equipment/supplies for and appropriately assists patients with oral and personal hygiene
3. Prepares patients for meals/ snacks/ liquids and assists with feeding, as needed.Assists in health care team's physical/psychological assessment of patient
4. Accurately measures and records patient's TPR, blood pressure, height, weight, intake/output and bowel movement, as appropriate
5. Collects required specimens; e.g., urine, feces, sputum
6. Reports any observable changes in patient's condition, treatment results, and/or patient/family concerns to the registered professional nurse.Performs selected diagnostic tests and prescribed treatments consistent with established procedures under supervision of registered nurse
7. Applies simple dressings
8. Discontinues intravenous peripheral therapies
9. Performs electrode application and EKG
10. Obtains accucheck results and informs nurse
11. Assists with the insertion of Foley catheters
12. Cleanses pressure injuries
13. Removes foley catheters according to established procedure
14. Correctly applies buck traction to patient
15. Assists patients with elimination, as required, e.g bedpans, urinals or enemas.Participates in health care team's responsibility to assure patient safety
16. Promptly responds to any patient's call bell and/or emergency light
17. Recognizes unsafe acts/conditions and corrects and/or reports same, as appropriate
18. Demonstrates appropriate use of side rails, restraints, gerichair and appropriately positions patients
19. Appropriately transports patients
20. Appropriately responds to fire, evacuation, and internal/external emergencies and drills.Functions effectively within organizational structure
21. Accepts assignment and seeks clarification, as needed
22. Carries out assignment
23. Reports patient care observations and findings to RN regularly.Participates as an integral part of the nursing team in assuming additional duties and responsibilities
24. Maintains competence by attending in-service education programs.Performs related duties, as required.
