Nashvillecenterrehab in Tennessee is seeking a Unit Manager to supervise nursing staff, coordinate admissions, and ensure 24/7 continuity of care across units. The role requires clinical leadership, timely documentation, and adherence to HIPAA and resident rights.

You will work with physicians and other health professionals to deliver resident-centered care, monitor staff performance, and maintain high-quality outcomes while supervising Charge, Treatment, and CNA assignments.

Qualifications

  • Must have graduated from an accredited Nursing college or university.
  • Must have a valid professional RN or LPN license for state of TN
  • Experience with long-term care and/or geriatric care is preferred but not limited to.
  • Well versed with documentation writing
  • Document patient's progress on weekly progress note in a timely fashion.
  • Collaborates with all disciplines to provide best patient centered treatment and care.

Responsibilities

  • Supervise, evaluate, counsel, and discipline personnel on assigned units.
  • Ensure continuity of care 24/7; receive and forward reports between shifts.
  • Identify issues and keep Nursing leadership informed; communicate status both verbally and in written reports.
  • Deliver outcome-focused nursing care using the nursing process; contribute to discharge planning.
  • Be point person for admissions during off hours, including tours and welcoming families.
  • Implement care plans effectively, with focus on resident-centered outcomes.
  • Collaborate with physicians and other health professionals to update plans of care.
  • Provide high-quality resident care and comfort with current nursing practices.
  • Supervise and coordinate CNA and other staff assignments; ensure timely communication of care information.
  • Monitor staff activities to ensure care conforms to plans and physician orders.
  • Demonstrate exceptional patient service and clinical excellence.

Job description

Unit Manager
  • Responsible for supervising, evaluating, counseling, and disciplining personnel on assigned units.
  • Responsible for ensuring continuity of care for 24 hours, 7 days a week. Receive report from previous supervisor for all nursing units, and forward report to oncoming supervisor prior to leaving tour of duty. Follow-up all identified issues.
  • Responsible for problem identification and resolution at the unit level. Keep the Director of Nursing/Associate Director of Nursing informed of residents' status through verbal communication and written report.
  • Assure that effective quality nursing care is delivered which is outcome focused through utilization of the nursing process. Identify needs and provide input for discharge planning and follow-up throughout length of stay
  • Be point person for admissions during off hours, including providing tours, welcoming new families and residents, ensuring a smooth and comfortable admission experience.
  • Implement plan of care consistently, effectively and cost efficiently with focus on resident centered outcomes
  • Work in collaboration with physician and/or other health care professionals by sharing information relevant to changing plan of care.
  • Deliver and maintain optimum resident care and comfort by demonstrating knowledge and skills of current nursing practices.
  • Will supervise and coordinate Charge, Treatment and CNA assignments and activities in an assigned unit and shift. Will ensure timely communication of current treatment & assessment information, efficient delivery of services and timely compilation of new information.
  • Will monitor and evaluate staff activities to ensure that nursing care is carried out as directed, and treatment administered in accordance with treatment plans and physician's instructions.
  • Exceptional patient service skills and ability to demonstrate clinical excellence
  • Report to work on time, adhere to scheduled hours and demonstrate professionalism at all times.
  • Compliance with confidentiality, HIPPA and Federal Residents Rights.
