Trinity Health is seeking a Nurse Senior – Complex Care Specialist to coordinate highly complex inpatient care within the Acute Care Management team. The role emphasizes leadership, care planning, and seamless transitions for acutely hospitalized patients with multifaceted needs.

Embedded within the Acute Care Management team, this role provides sustained attention, advanced care coordination, and accountability for complex inpatient populations, partnering closely with care teams and leadership to remove barriers to progression, support timely discharge planning, and improve patient outcomes.
While the role does not assume ownership of outpatient settings or teams, it is accountable for ensuring intentional coordination, connection, and effective handoff to appropriate outpatient, post‑acute, and population‑based resources as part of transitions of care.

Drives accountability for clear next steps, assigned responsibilities, and realistic timelines, partnering with providers and leadership to develop the safest possible plan of care and discharge.
Coordinates complex inpatient care through discharge by ensuring timely connection, clear communication, and effective handoff to outpatient, post‑acute, and population‑based resources, promoting safe and seamless transitions across care settings.
Applies advanced knowledge of workflows, payer and regulatory requirements, and community resources to identify system‑level barriers, partnering with leadership and external stakeholders to resolve delays and support efficient progression of care.
Maintains advanced knowledge of internal workflows, payer requirements, regulatory expectations, and community resources impacting complex acute care patients.
Identifies systemic gaps or recurring barriers contributing to extended length of stay or delayed discharge and works collaboratively with care teams and leadership to address them.
Engages external partners, community agencies, and payers as needed to support sustainable inpatient and post discharge care solutions.
May support or temporarily assume Case Management functions based on patient volume, acuity, complexity, and acute inpatient operational needs.

Must possess a comprehensive knowledge of nursing as normally obtained through a Bachelor’s degree in Nursing or a related field, and three (3) years of experience working with complex, high risk patient populations in an acute or inpatient setting.
Masters Degree in related field preferred.
Current Iowa RN license, (MercyOne Genesis, excluding Aledo and DeWitt, - current Registered Nurse (RN) license in either Iowa or Illinois is required at the time of hire. A secondary RN license for the other state must be obtained within 12 months of the hire date.)
Proof of completion of Mandatory Reporter – Child and/or Dependent Adult Abuse training within three (3) months of hire.
Colleagues employed and working solely in facilities located in Illinois are exempt from this requirement in accordance with applicable state requirements.
Experience leading complex cases, care planning discussions, or interdisciplinary efforts without formal supervisory authority.
Demonstrated success coordinating care for patients with significant medical, psychosocial, behavioral, and system complexity during hospitalization.
