Job summary

OPCO Skilled Management is seeking a dedicated Registered Nurse or Licensed Vocational Nurse to lead resident care at Gallup Nursing & Rehabilitation. The RN/LVN will develop care plans, collaborate with physicians and therapies, and supervise nursing staff to maintain safety standards.

The role requires active state licensure (RN or LVN) and current BLS/CPR certification. Prior nursing management experience is preferred, with duties including charting, narcotics oversight, and rounds with

Responsibilities

• Follows established procedures for charting and incident reporting.
• Follows procedures for narcotic counting, pharmacy orders, and restraint reduction.
• Documents changes in patient conditions including nutrition, infections, skin conditions, weight loss, etc.
• Utilizes the Quality Life Manual for patient placement and monitoring.
• Document and notify departments of patient room changes, LOA, admits, or discharges.
• Makes rounds with physicians when necessary.

The Registered Nurse or Licensed Vocational Nurse will be responsible for resident care and directing nursing staff. The RN is expected to develop the plan of care and collaborate with physicians, therapists and nursing assistants. Charge Nurse (RN or LVN) will provide nursing care within scope of practice to ensure patient’s needs are met in accordance with standards of practice, physician orders, center policies as well as state, federal and local guidelines.

QUALIFICATIONS

• Current LVN/LPN or RN license in practicing state.
• Current BLS/CPR certification or willingness to acquire one.
• Experience in nursing management is preferred but not required.

RESPONSIBILITIES

• Follows established procedure for charting and incident reporting.
• Follows company procedures for narcotic counting, pharmacy orders, and restraint reduction.
• Documents any change in patient's conditions including but not limited to nutritional problems, infections, skin conditions, weight loss or any other noticeable changes.
• Utilizes the Quality Life Manual for identification, placement, and monitoring of patients in the appropriate programs.
• Document and notify applicable departments of any patient's room changes, LOA, new admits, or discharges.
• Makes rounds with physicians when necessary.
