Evolutionary Healthcare is seeking compassionate Nurse Practitioners (NPs) and Physician Assistants (PAs) to join our growing team providing primary care services throughout post-acute and skilled nursing facilities in Central Florida. This role offers a chance to build meaningful relationships with patients and practice comprehensive medicine.

Qualifications

• Current Nurse Practitioner or Physician Assistant license in Florida.
• National Board Certification.
• DEA license (or ability to obtain).
• Excellent clinical judgment and communication skills.
• Experience in Internal Medicine, Family Medicine, Geriatrics, Hospital Medicine, Emergency Medicine, or Post-acute Care preferred.
• Skilled Nursing Facility experience is a plus, but not required.
• Commitment to compassionate, patient-centered care.

Responsibilities

• Provide comprehensive primary care services in skilled nursing and post-acute care facilities.
• Evaluate, diagnose, and manage acute and chronic medical conditions.
• Develop individualized treatment plans that improve function, mobility, and quality of life.
• Perform routine follow-up visits, admissions, discharge planning, and transitional care.
• Collaborate closely with nursing staff, therapists, specialists, and facility leadership.
• Educate patients and families regarding disease management and preventive care.
• Maintain accurate and timely documentation using electronic health records.

What You'll Do
