Job summary

Presbyterian Healthcare Services in Albuquerque, NM is seeking a CNS or NP to serve as a clinical resource, diagnose and manage acute and chronic diseases, and act as an independent practitioner across the care continuum.

The role features a Monday–Friday schedule from 7:30 am to 4:00 pm, no call obligation, and eligibility for student loan forgiveness under PSLF and NM programs, with opportunities to contribute to research and program development.

Qualifications

  • Master’s degree in Nursing is required.
  • Current NM advanced practice nursing CNS or NP license required.
  • Three to five years of nursing experience with one to three years in wound care.
  • Wound Specialist certification within one year of hire.
  • Case management/home care experience preferred.
  • NM Board of Pharmacy license, DEA, and CNS or NP licensure required.

Responsibilities

  • Compile and analyze data to independently formulate differential diagnosis, plan of medical management/plan of care, and monitor and evaluate outcomes for patients served.
  • Order and interpret diagnostic tests, prescribe necessary treatments and medications, perform minor surgical procedures, and provide referrals to other disciplines as needed.
  • Perform health maintenance and restoration activities meeting emotional, physical, and medical needs of clients, managing acute and chronic diseases.
  • Work autonomously and collaboratively with the healthcare team when issues fall beyond APRN scope; delegate appropriately.
  • Coordinate care plans with patients, families, and related disciplines to facilitate quality care and optimal outcomes; participate in interdisciplinary care conferences and serve as patient advocate.
  • Support superior outcomes through participation in job-related projects such as research, data collection, quality assurance, policy and procedures development, teaching activities, and program development to improve care quality and evaluate effectiveness.
  • Serve as a role model promoting excellence in practice by consistently using evidence-based practice principles; serve as a resource to other providers/disciplines.
  • Perform other functions as required.

Summary

The CNS or NP serves as a clinical resource and an independent practitioner who diagnoses and manages acute and chronic diseases in the area of advanced practice licensure. Proactively manages the care of patients across the continuum of care and oversees complex patient care.

Practice Highlights
