Qualifications

• Active New York State Nurse Practitioner License.
• DEA Registration (or ability to obtain).
• Strong clinical assessment and communication skills.
• Experience in: Family Medicine, Internal Medicine, Geriatrics, Skilled Nursing, Rehabilitation, Long‑Term Care, Sub‑Acute Care.

Responsibilities

• Performing routine and follow‑up medical evaluations.
• Managing chronic and acute medical conditions.
• Collaborating with physicians and interdisciplinary care teams.
• Developing individualized treatment plans.
• Reviewing labs, diagnostics, and medications.
• Communicating with patients, families, and nursing staff.
• Helping to prevent unnecessary hospitalizations.
• Focusing on delivering compassionate, patient‑centered care.
• Completing documentation with the support of our medical scribes.
