Job summary

Puzzle Healthcare, a national post-acute care organization, seeks a Nurse Practitioner in the Peru, IL area to provide focused care in nursing facilities, reducing readmissions and supporting recovery with autonomy and strong team backing.

You will round in facilities on a weekday daytime schedule, conduct micro visits, collaborate with physicians and staff, document in Puzzle’s EHR, and monitor readmission risk.

Qualifications

• Licensed NP with board certification in family or adult-gerontology.
• Experience in post-acute or long-term care preferred.
• Strong clinical assessment and communication skills.

Responsibilities

• Round in assigned facilities on a consistent weekday daytime schedule.
• Oversee daily patient interactions with focused assessments to manage pain, wounds, and functional decline.
• Collaborate with physicians, therapists, and facility staff on recovery plans.
• Document care in Puzzle Healthcare's EHR, often remotely from home.
• Monitor readmission risk and address gaps in post-acute care.
• Maintain compliance with post-acute and PM&R regulations.
