Job summary

Community Health Network seeks an Advanced Practice Provider to join our Employer Health Onsite primary care team at University of Indianapolis. The role focuses on the well-being of onsite employees and their beneficiaries, delivering routine check-ups, preventive care, and coordinated care within a large integrated health system.

Minimum two years of practice as an APP are required; Indiana licensure and family medicine certification preferred.

Qualifications

• Two years of experience as an Advanced Practice Provider required.
• Educational background consistent with licensure and certification in Indiana.
• Family medicine certification preferred for onsite employer health role.

Responsibilities

• Perform routine check-ups and preventive care.
• Diagnose and treat illnesses or injuries.
• Manage chronic or complex health conditions.
• Manage medications for patients and coordinate therapies.
• Refer to specialists for more serious conditions as needed.
