Primary Care Provider (PCP) Nurse Practitioner Responsibilities Overview

The PCP Nurse Practitioner demonstrates accountability for outcomes, strong clinical care, and cost-effectiveness for each patient in their panel of up to 450 patients. They understand that they can strongly influence the patient’s outcomes by building a trusting relationship and helping them change behaviors.

  • Coaches for health: acts as a health coach, helping patients set short and long-term health goals, partnering with the patient to work toward the goals, and frequently following up on those goals on the path to improved health.
  • Simplifies for action: simplifies and prioritizes appropriately so that behavior change is more actionable for the patient and for the care team.

We are an outcomes-focused, value-based organization and for their panel of patients, the following metrics are regularly measured to help PCPs become successful and reach partnership status: patient admissions per thousand, using between 18-21 appointment slots per day (each new patient counts for 2 slots, follow-up patients 1 slot), CGCAHPS (patient experience), clinical gaps closures, and medical cost measures. Each PCP will have goals for these metrics and will be expected to work toward those targets with their center and market leadership as well as their care teams. Culture is very important in the medical centers and because PCPs are leaders in our organization and centers, they are expected to help champion a positive culture of love, accountability, and passion along with center leadership.

The PCP will be required to demonstrate the ability to function both independently and in collaboration with other health care professionals. The PCP will work closely with the applicable managers and medical directors to ensure compliance with guidelines along with participating in risk and quality management programs, clinical meetings and other meetings as required that promote patient health and company goals.

The PCP will adhere to strict departmental goals/objectives, standards of performance, regulatory compliance, quality patient care compliance, and policies and procedures.

ESSENTIAL JOB DUTIES/RESPONSIBILITIES
  • Independently provides care for patients with acute and chronic illnesses encountered in older adults.
  • Take full accountability for patient care and outcomes and seek consultation from specialists when needed while staying involved in the detailed care of the patient.
  • Engage with hospitalists whenever one of their patients is in the hospital, regardless of hospital affiliation.
  • Responsible for assessment, diagnosis, treatment, management, education, health promotion and care coordination and documentation for patients with acute and complex chronic health needs.
  • Lead their care team consisting of care promoter (medical assistant), care facilitator, and care coordinator for patients able to come to the office.
  • For patients unable to come to the office—engage with the transitional care team and others including case managers, acute and transitional-care physicians, and other resources available depending on the market.
  • Have an active role in the management of their center and help cover for other providers who may be out for various reasons. Each PCP is also expected to take an active role as needed in recruiting patients for the center and additional providers for the company.
  • Supervise, collaborate with, participate with, or function within a practice or collaborative agreement with an Advanced Practice Practitioner (APP) and remain accountable for the actions of the APP while employed with the company.
  • Perform other duties as assigned and modified at manager’s discretion.
KNOWLEDGE, SKILLS & ABILITIES
  • Availability and Accessibility – Make themselves available to patients by being open for walk-in visits and responding to phone calls and messages in a timely manner.
  • Service Orientation – Provide care that would be desired for a family member or for themselves at every interaction.
  • Evidence Based Medicine – Remain updated on evidence-based medicine and on lifestyle and nutrition interventions to improve outcomes.
  • Physician Leadership – Continuously develop leadership skills for the benefit of patients, the team, the center, and the company.
  • Quality – Work with care teams towards achieving high quality outcomes and quality measures, and continuously improve practice and center operations.
  • Influence – Competently and compassionately influence patients, teams, and themselves to achieve the best outcomes.
  • Self-Care – Maintain physical, mental, social, and spiritual well-being to provide the best care to patients.
  • Proficient in Microsoft Office Suite (Word, Excel, PowerPoint, Outlook) and other related software.
  • Ability and willingness to travel locally as needed, nationally for initial training (2 weeks), and occasionally regionally and nationally for recruiting or training purposes.
  • Fluency in English, verbal and written. Additional language fluency may be required at certain centers.
  • Demonstrate independent judgment in performing duties.
EDUCATION AND EXPERIENCE CRITERIA
  • Current Nurse Practitioner Certification in the state of practice required.
  • Certification in Basic Cardiac Life Support required.
  • Board certification by AANP or ANCC preferred but may be required for certain states.
  • Current DEA number for schedule II-V controlled substances may be required based on state of practice.
  • 1 year of clinical experience preferred; strong new graduates considered.
PAY RANGE

$111,140 - $158,771 Salary

The posted pay range represents the base hourly rate or base annual full-time salary for this position. Final compensation will depend on a variety of factors including but not limited to experience, education, geographic location, and other relevant factors. The position may also be eligible for bonuses or commissions.
