Job summary

Gilder Search Group is seeking a dedicated Nurse Practitioner for a skilled nursing facility in Elizabethtown, KY. The NP will manage a panel of long-term care and short-stay residents, providing comprehensive primary care, chronic and acute disease management, and close coordination with nursing and therapy teams.

The role is full-time, in-person, Monday through Friday daytime hours with no nights or weekends. Licensure, DEA registration, and board certification are required.

Qualifications

• Active and unrestricted Kentucky NP license in good standing.
• Active DEA registration.
• Board certification as an FNP or AGNP.
• Experience in skilled nursing, long-term care, or geriatrics preferred.

Responsibilities

• Provide primary care to residents, including physical exams, assessments, and medication management.
• Diagnose and treat chronic and acute conditions within the NP scope of practice.
• Order and interpret labs and diagnostic tests, and adjust care plans accordingly.
• Collaborate with facility nursing staff, therapy, and the interdisciplinary team.
• Maintain accurate and compliant clinical documentation.
