Job summary

Avir Health Group is seeking a qualified Nurse Practitioner (Skilled Nursing) to join our family in a SNF setting. You will lead clinical care with the facility's care team, overseeing admissions, discharges, monthly stability checks, and condition-change evaluations.

Responsibilities include advanced care planning, comprehensive assessments, and prescribing treatments. Requirements include an active Texas APN licensure and DEA license, prior nursing/clinical experience, and strong communication

Qualifications

• Master’s degree in Nursing required.
• Strong communication and teamwork skills essential.
