Avir Health Group is seeking a Nurse Practitioner (Skilled Nursing) to lead a clinical team in a Skilled Nursing Facility. You will oversee admissions, discharges, monthly stability checks, and evaluations for changes in condition while guiding advanced care planning and comprehensive assessments.

With an emphasis on collaborative practice, you will prescribe treatments and work with the care team to ensure high-quality resident care, focusing on short- and long-term needs in a supportive SNF

Qualifications

• Active Texas APN licensure required.
• DEA license required.
• Previous nursing/clinical experience required.
• Master’s degree in Nursing required.
