Qualifications

• APN with national certification and prescriptive authority
• Experience in primary care and chronic disease management preferred
• Ability to work collaboratively with an interdisciplinary team

Responsibilities

• Independently manages a panel of patients with acute and chronic conditions
• Develops and implements individualized plans to improve population health
• Orders and interprets labs and diagnostic studies; prescribes pharmacologic and non-pharmacologic interventions
