Job summary

Curana Health, Inc. seeks an experienced Psychiatric Nurse Practitioner (PMHNP) to deliver compassionate mental health care across SNF, LTC, AL, and IL communities. You will work with a multidisciplinary team to ensure continuity of care and individualized treatment plans.

This full-time role requires travel between facilities; offer includes a $10k sign-on bonus, competitive compensation, comprehensive benefits, and opportunities for CME and professional growth.

Qualifications

• Graduation from an accredited Nurse Practitioner program and ANCC certification as a PMHNP.
• Active DEA license.
• BLS and ACLS certification.
• Current NP license in the practicing state.

Responsibilities

• Conduct rounds and manage psychiatric care in SNF, LTC, AL, and IL settings.
• Collaborate with physicians, specialists, referral sources and staff for coordinated care.
• Assess and manage residents' mental health needs and perform physical exams.
• Order and interpret labs and tests; develop treatment plans for mental and physical health.
• Prescribe medications and provide treatments; administer injections as needed.
• Educate patients and families on medications, care plans, and preventive mental health practices.
• Coordinate acute psychiatric and medical care with facility teams.

Essential Duties & Responsibilities

• Psychiatric Care and Rounding: Conduct patient rounds and manage psychiatric care within SNF, LTC, AL, and IL settings.
• Collaborative Care Coordination: Partner with attending physicians, consulting specialists, referral sources, and facility staff to ensure comprehensive, coordinated care.
• Patient Evaluation and Management: Assess and manage the mental health needs of residents, perform physical exams, and gather complete medical histories.
• Diagnostics and Therapeutics: Order and interpret lab and diagnostic tests, make preliminary diagnoses, and develop effective therapeutic care plans for mental and physical health.
• Medication and Treatment Management: Prescribe and administer medications and treatments; provide injections as necessary.
• Patient and Family Education: Educate patients and families on medications, care plans, and preventive mental health practices.
• Acute Care Collaboration: Work alongside physicians and facility teams to address acute psychiatric and medical needs promptly and effectively.
