Job summary

Qualifications

• BLS certification is required.

Responsibilities

• Provide outpatient psychiatric evaluations, diagnosis, and medication management five days a week.
• Treat mental health and substance use disorders with evidence-based practices including MAT.
• Initiate and manage FDA-approved medications for SUDs per guidelines and regs.
• Monitor treatment responses and adjust plans accordingly.
• Collaborate with therapists, PCPs, case managers, and recovery services for integrated care.
• Develop individualized treatment plans incorporating meds, counseling, and therapies.
• Maintain compliant documentation and prescribing practices with regulations and payer rules.
• Deliver education and clinical support to staff and learners.

Duties and Responsibilities

• Provide comprehensive psychiatric evaluations, diagnosis, and medication management for adult and pediatric patients in an outpatient setting five (5) days per week.
• Assess and treat mental health and substance use disorders using evidence-based practices, including Medication-Assisted Treatment (MAT).
• Initiate, prescribe, and manage FDA-approved medications for opioid, alcohol, and other substance use disorders in accordance with current clinical guidelines and applicable federal and state regulations.
• Monitor treatment response through ongoing follow-up visits, evaluating medication effectiveness, adherence, withdrawal symptoms, cravings, side effects, and overall progress, adjusting treatment plans as clinically indicated.
• Collaborate with behavioral health therapists, primary care providers, case managers, community partners, and recovery support services to provide comprehensive, integrated care for patients with substance use disorders.
• Develop individualized treatment plans that incorporate medication management, counseling, behavioral therapies, and recovery support services.
• Maintain documentation and prescribing practices that comply with all federal and state regulations, payer requirements, and Health Partnership Clinic policies related to Medication-Assisted Treatment.
• Develop, implement, and monitor individualized treatment plans in collaboration with patients, families, and the interdisciplinary care team.
• Order, interpret, and monitor appropriate laboratory studies and diagnostic tests necessary for safe psychiatric medication management.
• Identify patients requiring higher levels of care and facilitate referrals to specialty psychiatric services, specialty addiction services, crisis services, or inpatient treatment when clinically indicated.
• Maintain timely, accurate, and complete electronic medical records.
• Contribute to quality improvement initiatives and support organizational performance measures, including HRSA and value-based care initiatives.
• Provide education and clinical support to staff and learners, as requested.
• Maintain required licensure, certification, continuing education, and credentialing requirements.
• Follow all Health Partnership Clinic policies, procedures, clinical protocols, and regulatory requirements.
• Participate in required agency education, training, and competency assessments.
• Perform other duties as assigned.
• Regular attendance and punctuality are essential functions of this position.

Qualifications

• Must be certified as a Psychiatric Mental Health Nurse Practitioner.
• Must possess an active KS license as a Psychiatric Mental Health Nurse Practitioner and maintain an active and unrestricted KS DEA number.
• Must be eligible for liability coverage via FTCA and private malpractice insurance carrier.
• At least two (2) years of experience within a community health center or similar environment preferred.
• Must maintain BLS certification.
• Staff membership and privileges granted by the HPC Board of Directors (approval of credentialing and privileging applications).
• Ability to relate well with patients and other health care personnel.
• Social and cultural sensitivity appropriate to ethnically and economically diverse patient populations and employee base.
• Willingness to educate patients and the community in health maintenance and the role of the health care team.
• Commitment to expanding access to care for the underserved.

Typical Physical Demands

• Prolonged sitting or standing may be required.
• Ability to lift 25 pounds; occasionally up to 60 pounds.
• Those physical movements and the degree of mobility, manual dexterity and hand-eye coordination normally associated with normal medical office will be performed on a repetitive basis.
• The ability to distinguish letters and symbols as well as the ability to utilize telephones, computer terminals and copiers is required.
• Work under stressful conditions as well as irregular hours may be required.
• Frequent exposure to communicable disease, body fluids, toxic substances, medicinal preparations, radiation, and other conditions common to a clinical environment may routinely be encountered.
