Job summary

Hueman is actively hiring Nurse Practitioners to complete in-home Health Risk Assessments (HRAs) for Medicare and Medicaid members in and around Laconia, NH. This role offers flexibility to fit your lifestyle while delivering high-quality care in patients’ homes.

Qualified NPs will conduct non-invasive assessments, review medical histories, and document findings while maintaining patient safety and education. Travel is required for home visits with comprehensive training provided by Hueman.

Qualifications

• Master’s Degree in nursing or commensurate experience with NP license.
• Active NP license in the relevant state.
• Active national board certification for NP.
• Current BLS certification.
• 12 months of paid advanced clinical provider experience is preferred but not required.

Responsibilities

• Travel to and from patients' homes.
• Perform comprehensive, non-invasive physical assessments.
• Review medical history and ensure accurate and timely documentation.
• Conduct medical reconciliation to ensure patient safety and compliance.
• Provide basic patient education on wellness and preventative healthcare measures.

Education

12 months of paid advanced clinical provider experience (preferred)

Position Summary

Qualified Nurse Practitioners interested inthispositionwill have the opportunity to interact with members in a more intimate setting. Unlike a typical clinic visit, this role allows NPs to connect with patients on a more personal and effective level. Nurse Practitioners performing HRAs are not required to treat or prescribe in the home.
