Hueman in Gloster, MS is hiring Nurse Practitioners to perform in-home Health Risk Assessments for Medicare and Medicaid members. Travel to patients’ homes to conduct non-invasive physical exams, review medical history, and document findings accurately.

This role offers a flexible schedule, comprehensive training, and support such as scheduling assistance and travel reimbursement. NPs are not required to treat or prescribe in home clinics.

Qualifications

• Master’s degree in nursing or commensurate experience with NP licensure.
• Active State NP license required.
• Active national board certification as NP.
• Current BLS certification required.
• 12 months of paid advanced clinical provider experience preferred.

Responsibilities

• Travel to and from patients’ homes for HRAs.
• Perform comprehensive, non-invasive physical assessments.
• Review medical history and document accurately and timely.
• Conduct medical reconciliation to ensure patient safety.
• Provide basic patient education on wellness and preventative care.

Position Summary

Qualified Nurse Practitioners interested in this position will have the opportunity to interact with members in a more intimate setting. Unlike a typical clinic visit, this role allows NPs to connect with patients on a more personal and effective level. Nurse Practitioners performing HRAs are not required to treat or prescribe in the home.

Nurse Practitioner Responsibilities

• Travel to and from patients’ homes.

• Review medical history and ensure accurate and timely documentation.
• Conduct medical reconciliation to ensure patient safety and compliance.
• Provide basic patient education on wellness and preventative healthcare measures.

Nurse Practitioner Qualifications and Experience Requirements

• Master’s Degree OR commensurate experience and satisfactory completion of NP license.
• Active State NP license.
• Active National board certification.
• Current BLS certification.
• 12 months of experience as a paid advanced clinical provider is preferred but not required.
