Job summary

Hueman is actively hiring Nurse Practitioners to perform in-home Health Risk Assessments for Medicare and Medicaid members in and around Elberton, GA. This role offers flexible scheduling and travel to patients’ homes, with comprehensive training to ensure accurate documentation and quality patient interactions.

NPs will conduct non-invasive physical assessments, review medical histories, and provide basic wellness education. Licensing requires an active NP license and board certification.

Qualifications

• Master’s degree in nursing or commensurate experience and NP license.
• Active State NP license.
• Active national board certification.
• Current BLS certification.
• Preferred: 12 months of paid NP experience.

Responsibilities

• Travel to and from patients’ homes.
• Perform comprehensive, non-invasive physical assessments.
• Review medical history and ensure accurate documentation.
• Conduct medical reconciliation for patient safety and compliance.
• Provide basic patient education on wellness and preventative care.

Position Summary

Qualified Nurse Practitioners interested inthispositionwill have the opportunity to interact with members in a more intimate setting. Unlike a typical clinic visit, this role allows NPs to connect with patients on a more personal and effective level. Nurse Practitioners performing HRAs are not required to treat or prescribe in the home.

• Travel to and from patients’ homes.

• Conduct medical reconciliation to ensure patient safety and compliance.
• Provide basic patient education on wellness and preventative healthcare measures.

Nurse Practitioner Qualifications and Experience Requirements:

• Master’s Degree OR commensurate experience and satisfactory completion of NP license
• Active State NP license
• Active National board certification
• Current BLS certification
• 12 months of experience as a paid advanced clinical provider is preferred but not required
