Job summary

Hueman is actively hiring Nurse Practitioners to perform in-home Health Risk Assessments for Medicare and Medicaid members in Caro, MI. NPs will travel to patients' homes and conduct non-invasive assessments in a flexible schedule designed around personal goals.

Qualified NPs will hold a state license, board certification, and BLS, with prior paid clinical provider experience preferred. Training is provided, and liability insurance options are included to support caregivers.

Qualifications

• Master's Degree or commensurate experience and NP license are required.
• Active National board certification is required.
• Current BLS (Basic Life Support) certification is required.

Responsibilities

• Travel to and from patients' homes to perform HRAs.
• Conduct comprehensive, non-invasive physical assessments.
• Review medical history and document findings accurately and timely.
• Perform medical reconciliation to ensure patient safety and compliance.
• Provide basic patient education on wellness and preventive care.

Position Summary

Qualified Nurse Practitioners interested inthispositionwill have the opportunity to interact with members in a more intimate setting. Unlike a typical clinic visit, this role allows NPs to connect with patients on a more personal and effective level. Nurse Practitioners performing HRAs are not required to treat or prescribe in the home.

Nurse Practitioner Responsibilities

• Travel to and from patients homes.
• Perform comprehensive, non-invasive physical assessments.
• Review medical history and ensure accurate and timely documentation.
• Conduct medical reconciliation to ensure patient safety and compliance.
• Provide basic patient education on wellness and preventative healthcare measures.

Nurse Practitioner Qualifications and Experience Requirements:

• Masters Degree OR commensurate experience and satisfactory completion of NP license
• Active State NP license
• Active National board certification
• Current BLS certification
• 12 months of experience as a paid advanced clinical provider is preferred but not required
