Job summary

Shaker Medical Group in New Jersey is seeking an Advanced Nurse Practitioner to provide primary care across life stages in outpatient and rehab settings. You will perform exams, take histories, order tests, and manage medications within NP scope while collaborating with a multidisciplinary team.

The role emphasizes continuity of care, health coaching, and transitions of care between rehab facilities and outpatient offices, with a focus on quality patient outcomes.

Qualifications

• NJ NP license with prescriptive authority required.
• Master's degree in Nursing with NP specialization.
• NP certification in Family, Adult-Gerontology or Pediatrics.
• Experience in primary care preferred.

Responsibilities

• Manage acute and chronic conditions with continuity across care settings.
• Provide primary care across lifespan in outpatient and long-term care settings.
• Perform physical exams and take detailed histories.
• Order and interpret diagnostic tests to guide decisions.
• Prescribe and manage medications within NP scope.
• Conduct health coaching and preventive care education.
• Collaborate with physicians, therapists, nurses and other staff.
• Support transitions of care between rehab, nursing facilities and office.
