Job summary

Providence Community Health seeks a Primary Care Clinician for patients experiencing homelessness at a brand-new PCHC Broad Street clinic in Providence, RI. You will work with clinicians to deliver comprehensive primary care aligned with policies and standards, improving care quality for a vulnerable population.

Requirements include RN and Nurse Practitioner training, RI prescriptive privileges, and Family or Internal Medicine certification, plus at least two years in community health or

Qualifications

• RN and NP training completed at an accredited college or university.
• Licensed RN and NP with prescriptive privileges in Rhode Island.
• Certified as NP in Primary Care (Family or Internal Medicine).
• Continuing education to meet state/national standards.
• Minimum 2 years of community health or ambulatory care experience.

• Collaborate with health center staff to deliver efficient, comprehensive care.
• Improve quality of care in accordance with policies and standards.
• Support access and continuity of care for patients experiencing homelessness.
