Job description

JOB TITLE: Nurse Practitioner OR Physician Assistant
Schedule: M-F 20 hours/week
Pay: $70-75/hr
Location: Remote (Initial 60 day training in Pleasanton)

Role Overview:

As a Nurse Practitioner (NP) or Physician Assistant (PA), you will play a key role in ensuring the delivery of quality medical care. You will conduct health assessments, monitor patients, and prescribe withdrawal management medications, including Medication-Assisted Treatment (MAT). You’ll collaborate closely with the Medical Director to ensure clients receive the necessary care and refer them to specialists when appropriate.

In this role, you’ll help individuals with substance use challenges—often with co-occurring conditions—by providing compassionate care, crisis intervention, and de-escalation in a supportive and inclusive work environment. You’ll empower individuals to transition from hopelessness to a place of hope and potential.

Key Responsibilities:
  • Interpret vital signs collected by LVNs and other healthcare staff.
  • Review intake health information, including communicable diseases, to inform treatment.
  • Assess substance use history and drug test data to ensure safe stabilization.
  • Complete documentation accurately and promptly.
  • Monitor residents for physical or mental decompensation, escalating care when necessary.
  • Review and utilize withdrawal assessment scales (CIWA, COWS) to manage withdrawal symptoms.
  • Prescribe withdrawal management medications and MAT under MD supervision.
  • Report medical incidents or unusual occurrences to Program Manager, Compliance Manager, and MD.
  • Collaborate with the multidisciplinary team (MD, LVN, clinical staff) to provide quality care.
  • Maintain up-to-date treatment plans and progress notes in electronic health records.
  • Provide support when deviations from medication orders are requested.
  • Mentor healthcare staff and assist with understanding IMS services.
  • Attend all required meetings and maintain punctuality.
  • Maintain a clean, safe, and supportive environment.
