Job summary

Bluestone Physician Services is seeking an NP or PA to deliver on-site, team-based primary care in Assisted Living and Memory Care communities in Fort Myers, FL. You will manage a panel of about 200 patients and design care plans with clinical support from LPNs, RNs and social workers.

The role is full-time (Mon-Thu; potential Friday) with access to labs and imaging, remote collaboration as needed, and a strong emphasis on quality outcomes and patient-centric care.

Qualifications

• Graduate of an accredited NP or PA education program.
• Board certification or board eligibility through AANP/ANCC/NCCPA.
• Licensed or eligible for licensure via state boards.
• DEA certification as required by state of practice.
• Strong verbal communication and interpersonal skills.
• Valid driver's license and required auto insurance; local travel.

• Full-time schedule: Monday-Thursday; option for Fridays as clinical or administrative day.

Position Overview

• Provide on-site, team-oriented primary care to patients within Assisted Living and Memory Care communities
• Full-time providers care for patients Monday-Thursday; option for Fridays to be a clinical or an administrative day
• Manage a panel of 200 patients (approximately 12-16 patients per/day with 14 being the avg.)
• Ability to design and manage your schedule
• Develop and implement care plans appropriate to patients and their desires
• Clinical support from Clinical Assistants and Care Coordination through LPNs, RNs and Social Workers
• Access to diagnostics such as labs and x-rays as needed
• Collaborate with community partners within home care, hospice, and other resources as appropriate

Qualifications

• Graduate of an accredited Nurse Practitioner or Physician Assistant Education Program
• Board Certified/Board Prepared through AANP, ANCC, or NCCPA
• Licensed or ability to obtain licensure through the Board of Nursing or Board of Medical Practice
• Ability to obtain and /or maintain DEA certification for the State of required of work
• Proven ability to communicate effectively with strong verbal skills
• Excellent interpersonal and customer service skills
• Must have a valid and unrestricted driver's license and minimum state-required liability auto insurance; local travel is required

Preferred Qualifications

• 2+ years of clinical experience as a Nurse Practitioner or Physician Assistant
