Job summary

Bluestone Physician Services is seeking a Nurse Practitioner or Physician Assistant to provide on-site, team-based primary care in Assisted Living and Memory Care communities around Bonita Springs, FL. You will manage a patient panel, design care plans, and collaborate with a multi-disciplinary team to deliver proactive, high-quality care.

Applicants should be graduates of an NP or PA program, board-certified or eligible, and licensed to practice.

Qualifications

• Graduate of an accredited Nurse Practitioner or Physician Assistant program.
• Board certification or eligibility through AANP/ANCC/NCCPA.
• Licensed or ability to obtain licensure in the state.
• Ability to obtain and maintain DEA certification as required.

Responsibilities

• Provide on-site, team-based primary care to patients in Assisted Living and Memory Care communities.
• Manage a patient panel (approx. 200) with 12–16 patients per day.
• Design and manage your schedule to meet patient and organizational needs.
• Collaborate with clinical assistants, LPNs, RNs, and social workers for care plans.
• Access to diagnostics like labs and x-rays as needed.
• Coordinate with home care, hospice, and other community resources.

Join our growing practice as a Nurse Practitioner (NP) or Physician Assistant (PA) and bring exceptional team-based care directly to patients residing at Assisted Living and Memory Care communities in and around Bonita Springs, FL.

Position Overview:

• Provide on-site, team-oriented primary care to patients within Assisted Living and Memory Care communities
• Full-time providers care for patients Monday-Thursday; option for Fridays to be a clinical or an administrative day
• Manage a panel of 200 patients (approximately 12-16 patients per/day with 14 being the avg.)
• Ability to design and manage your schedule
• Develop and implement care plans appropriate to patients and their desires
• Clinical support from Clinical Assistants and Care Coordination through LPNs, RNs and Social Workers
• Access to diagnostics such as labs and x-rays as needed
• Collaborate with community partners within home care, hospice, and other resources as appropriate

Qualifications:

• Graduate of an accredited Nurse Practitioner or Physician Assistant Education Program
• Board Certified/Board Prepared through AANP, ANCC, or NCCPA
• Licensed or ability to obtain licensure through the Board of Nursing or Board of Medical Practice
• Ability to obtain and /or maintain DEA certification for the State required of work
• Proven ability to communicate effectively with strong verbal skills
• Excellent interpersonal and customer service skills
• Must have a valid and unrestricted driver’s license and minimum state-required liability auto insurance; local travel is required

Preferred Qualifications:

• 2+ years of clinical experience as a Nurse Practitioner or Physician Assistant
