Job summary

Theoria Medical is seeking a part-time Advanced Practice Provider in Pratt, KS, to serve local skilled nursing facilities within a dedicated care community. You’ll provide patient-centered care, conduct daily rounds, and build trusted relationships with residents and facility staff.

We offer flexible scheduling, autonomy, and a collaborative team, with technology support including an iPad for charting and access to clinical decision resources.

Qualifications

• Active U.S. license required and board certification for NP or PA accreditation.
• Specialties include Internal/Family/Emergency/Geriatric Medicine as applicable.

Responsibilities

• Provide high-quality, patient-centered care to residents in skilled nursing facilities.
• Conduct daily rounds and develop prescribed treatment plans.
• Address acute or emergent issues and adjust care as needed.
• Communicate with facility staff via phone or secure messaging.
• Maintain reliable on-site attendance for scheduled shifts.

What You’ll Do

• Provide high-quality, patient-centered care to residents in skilled nursing facilities.
• Conduct daily rounding, prescribe medications/treatment plans, and address acute/emergent issues.
• Communicate with facility nursing staff via phone or secure messaging as needed.
• Maintain reliable on-site attendance for scheduled shifts.
• Ensure compliance with clinical standards, facility policies, and regulatory requirements.

Your Qualifications

Advanced Practice Providers must hold an active U.S. license and be board-certified by ANCC, AANPCB, PNCB, or AACN (for Nurse Practitioners), or by NCCPA (for Physician Assistants). Qualified specialties include, but are not limited to, Internal Medicine, Family Medicine, Emergency Medicine, General Practice, Physical Medicine and Rehabilitation, and Geriatric Medicine.
