Job summary

Theoria Medical is seeking a part-time Advanced Practice Provider to serve local skilled nursing facilities, delivering high-quality, resident-centered care. You’ll round, manage treatment plans, and coordinate with facility teams, while enjoying flexible scheduling and independence within a collaborative clinical group.

Qualified applicants hold an active U.S. license and board certification (ANCC/AANPCB/PNCB/AACN or NCCPA for PAs) and can earn a competitive, productivity-based compensation

Responsibilities

• Provide high-quality, resident-centered care to SNF residents.
• Conduct daily rounds, manage medications and treatment plans.
• Communicate with facility nursing staff via secure channels as needed.
• Maintain reliable on-site attendance for scheduled shifts.
• Ensure compliance with clinical standards and facility policies.

What You’ll Do

• Provide high-quality, patient-centered care to residents in skilled nursing facilities.
• Conduct daily rounding, prescribe medications/treatment plans, and address acute/emergent issues.
• Communicate with facility nursing staff via phone or secure messaging as needed.
• Maintain reliable on-site attendance for scheduled shifts.
• Ensure compliance with clinical standards, facility policies, and regulatory requirements.
  

Your Qualifications

Advanced Practice Providers must hold an active U.S. license and be board-certified by ANCC, AANPCB, PNCB, or AACN (for Nurse Practitioners), or by NCCPA (for Physician Assistants). Qualified specialties include, but are not limited to, Internal Medicine, Family Medicine, Emergency Medicine, General Practice, Physical Medicine and Rehabilitation, and Geriatric Medicine.
