Theoria Medical is seeking a full-time Advanced Practice Provider (Nurse Practitioner or Physician Assistant) to serve local skilled nursing facilities in Brooklyn, OH. You will provide high-quality, patient-centered care to residents, with flexible scheduling and autonomy supported by a collaborative clinical team.

• Active U.S. license and board certification for NP or PA.
• Full-time availability to work in skilled nursing facilities.
• Experience in post-acute or SNF settings preferred.

• Conduct daily rounding, prescribe medications/treatment plans, and address acute/emergent issues.
• Communicate with facility nursing staff via phone or secure messaging as needed.
• Maintain reliable on-site attendance for scheduled shifts.
• Ensure compliance with clinical standards, facility policies, and regulatory requirements.

What You’ll Do

Your QualificationsAdvanced Practice Providers must hold an active U.S. license and be board-certified by ANCC, AANPCB, PNCB, or AACN (for Nurse Practitioners), or by NCCPA (for Physician Assistants). Qualified specialties include, but are not limited to, Internal Medicine, Family Medicine, Emergency Medicine, General Practice, Physical Medicine and Rehabilitation, and Geriatric Medicine.
