Theoria Medical in Lake Placid, FL is seeking a full-time Advanced Practice Provider (NP or PA) to serve local skilled nursing facilities within a close-knit, mission-driven care community. You will deliver high-quality, resident-centered care, with daily rounding and treatment planning, while collaborating with a supportive clinical team.

Qualifications

• Active U.S. license to practice in nursing or medicine.
• Board certification by ANCC, AANPCB, PNCB, or AACN (for NPs) or NCCPA (for PAs).
• Experience in post-acute care or SNF is preferred.

Responsibilities

• Provide high-quality, patient-centered care to residents in skilled nursing facilities.
• Conduct daily rounding and develop treatment plans for residents.
• Prescribe medications and manage acute/emergent issues as needed.
• Coordinate with facility nursing staff via secure channels.
• Maintain reliable on-site attendance for scheduled shifts.

What You’ll Do

• Provide high-quality, patient-centered care to residents in skilled nursing facilities.
• Conduct daily rounding, prescribe medications/treatment plans, and address acute/emergent issues.
• Communicate with facility nursing staff via phone or secure messaging as needed.
• Maintain reliable on-site attendance for scheduled shifts.
• Ensure compliance with clinical standards, facility policies, and regulatory requirements.

Your QualificationsAdvanced Practice Providers must hold an active U.S. license and be board-certified by ANCC, AANPCB, PNCB, or AACN (for Nurse Practitioners), or by NCCPA (for Physician Assistants). Qualified specialties include, but are not limited to, Internal Medicine, Family Medicine, Emergency Medicine, General Practice, Physical Medicine and Rehabilitation, and Geriatric Medicine.
