Job summary

Anpex Health in East Orange, NJ is seeking a Nurse Practitioner to join a mission-driven team delivering patient-centered care in a collaborative environment.

You will provide primary care with a focus on symptom management, conduct histories and exams for complex patients, collaborate with the interdisciplinary team, and support transitions while emphasizing comfort, dignity, and family education about disease trajectory and prognosis.

Qualifications

  • Master’s (MSN) or Doctorate (DNP) Nurse Practitioner program completion
  • Experience in primary care, geriatrics, or post-acute care preferred, but not required
  • Palliative care training and certification provided

Responsibilities

  • Provide primary care services focused on symptom management.
  • Conduct comprehensive history and physical exams for patients with complex acute, chronic, and serious illnesses.
  • Review abnormal vitals and laboratory findings and implement appropriate interventions or escalate to the attending physician as needed.
  • Collaborate with the interdisciplinary team (IDT) weekly to review patient status, prognosis, and care plans.
  • Support care transitions (admissions, transfers, discharges) with emphasis on continuity, comfort, and patient-centered goals.
  • Provide education and emotional support to patients and families regarding disease trajectory, prognosis, and end-of-life expectations.
  • Collaborate with multidisciplinary teams including nursing, social work, and therapy services.
  • Monitor effectiveness of interventions and adjust care plans to maintain comfort, dignity, and physiologic stability
