Job summary

Alignment Health is seeking a Field NP or PA to support the Care Anywhere program, delivering medical and social care to our senior patients in their homes. Responsibilities include 2 days per week in-home assessments, 2 days remote work, medication reviews, care planning, and collaboration with PCPs, specialists, and the regional medical officer to create the best possible plans for each member.

Requirements include a Master’s NP/PA degree, active California NP/PA license with board

The field remote NP or PA in the Care Anywhere (CAW) program is designed to provide care and support to our Alignment Healthcare patients by providing an additional level of medical and social support in the comfort of their own home. This program is offered to eligible patients at no cost to them with the goal of preventing unnecessary hospitalizations, health complications, and unmanaged disease progression that can occur when timely clinical interventions are not provided or are not accessible.

Responsibilities

1. 1. M-F schedule.
2. 2 days a week Conduct in-home assessments on Alignment members, medication review, health screenings and 3 days a week working from home.
3. 2. Provide patient education by matching care desired with best care given
4. 3. Identify diagnoses to be assessed in care management and active medical management
5. 4. Partner and communicate with Regional Medical Officer and other team members to discuss and develop the most appropriate care plans possible based on the needs of our members/patients.
6. 5. Coordinate care with multiple stakeholders, including but not limited to PCPs, specialists, and ancillary providers.
7. 6. Lead broader clinical team which may include nurses, health coaches, social workers, and care coordinators

Job Requirements

• Experience: Required: None Preferred: One (1) year of prior clinical or home care experience. Previous EMR experience preferred. Experience in care of older adult (geriatric) patients preferred
• Education: Required: Master’s degree from an accredited NP Program or PA program
• Specialized Skills: Required: Ability to communicate positively, professionally and effectively with others.
• Problem-Solving Skills: Effective problem solving, organizational and time management skills and ability to work in a fast-paced environment.
• Licensure: Required: Active Nurse Practitioner or Physician Assistant license for the state of California Active Nurse Practitioner Board Certification/Physician Assistant Certification NPI Number, DEA, Valid BLS Valid Driver License and current automobile insurance Preferred:

Pay Range: $130,332.00 - $195,498.00 Pay range may be based on a number of factors including market location, education, responsibilities, experience, etc.
