Job summary

Ennoble Care seeks an Advanced Provider (NP/PA) to deliver primary and palliative care to homebound patients in the Augusta, Georgia area on a PRN basis. You will apply medical principles under physician supervision to a diverse patient population with chronic conditions.

The role emphasizes independent practice, patient-centered care, and collaboration with a broader clinical team. A flexible PRN arrangement with strong patient impact is offered.

Qualifications

• Board certification as NP or PA in Georgia.
• Strong clinical skills for acute and chronic care.
• Positive demeanor and excellent interpersonal skills.
• Good problem-solving and independent work ability.
• 1–2 years in primary care with geriatric patients ( hospice a plus).

Responsibilities

• Provide high-quality primary and palliative care at patients' homes in and around Augusta, GA.
• Perform exams, gather histories, and develop care plans.
• Communicate effectively with clinic staff and operations platforms.
• Care for a diverse patient panel with chronic diseases.
• Prescribe medications and coordinate tests as needed.

Purpose

We are looking for an experienced Advanced Provider (Nurse Practitioner or Physician Assistant) to provide primary care to homebound and palliative care patients in Augusta, Georgia area on a PRN basis. You will apply medical techniques and principles under the supervision of a physician, and you will care for a diverse set of patients with chronic diseases.

What You'll Do

• Provide high quality care to primary and palliative patients in and around Augusta, GA. (Newnan County)
• Assess patient's health by performing physical examinations; obtaining, updating, and studying medical histories and creating the best plan for care.
• Effectively communicate with office staff and clinical operations across various internal platforms
• Provide high quality, patient centered care to a panel of patients
• Develop impressions, diagnosis, and treatment plans
• Foster trust and consult with patients, families, and caregivers during visits
• Order diagnostic testing as medically necessary
• Prescribe medications supplies and appropriate medically necessary interventions

What You'll Need

• Board Certified, with a valid NP or PA certification in the State of Georgia
• Strong clinical skills with the ability to accurately and confidently treat, assess, and diagnose
• Positive demeanor and strong people skills
• Ability to confidently problem solve and operate independently
• 1-2 years of primary care experience with geriatric patients (hospice experience highly preferred but not required)
