Summary:

At Presbyterian Healthcare Services, we're creating a different kind of primary care experience for some of our most vulnerable / at risk patients. We're seeking a compassionate and experienced provider to deliver highly personalized care to a medically complex population of dual-eligible Medicare and Medicaid patients.
This unique role is designed to provide the time, attention, and resources that traditional primary care settings often cannot. You'll focus on patients with multiple chronic conditions, unmet healthcare needs and treatable medical issues that require closer follow-up and more intensive care coordination.

Job Description:

As an Advanced Primary Care Provider, you will deliver comprehensive primary care services to a panel of medically complex patients who require a higher level of clinical attention and support.

Unlike traditional primary care settings, this role intentionally allows for smaller patient panels and longer appointment times, giving you the opportunity to fully assess patient needs, address barriers to care, coordinate resources, and improve overall health outcomes.

Patients in this program are those who would benefit from enhanced clinical management beyond what can typically be provided in a standard primary care practice.

Schedule
  • Monday through Friday
  • 8:00 AM to 5:00 PM
  • No weekends
  • No traditional high-volume clinic expectations
  • 12-14 patients per day
  • 30-60-minute appointment times
Key Responsibilities
