UF Health is seeking an experienced Advanced Practice Registered Nurse to coordinate patient care for heart transplant and heart failure patients in Gainesville, Florida. This role involves assessing patient needs, implementing nursing interventions, and ensuring continuity of care.
The ideal candidate will possess a Master's degree in nursing and at least three years of experience, preferably in critical care or transplantation. A current APRN license in Florida and BLS certification are required.
Ensures coordination and continuity of patient care for heart transplant and heart failure patients in order to maintain organized and efficient care and provide support to staff and other health care professionals. Responsible for identifying the patient's alterations in health; assessing the patient's response to treatment protocols; communicating appropriate data to physician staff; implementing nursing interventions based upon established treatment protocols; and anticipating potential outcomes for patients at risk for complications. Must demonstrate ability to assess data reflective of the patient's status and interpret the appropriate information needed to identify each patient's requirements relative to age‑specific needs and organ(s) transplanted and provide care according to protocols and within practice of nursing.
Schedule: M‑F 8a‑4:30p. Occasional call required from 4:30‑8a.
