Innovative Medical Group | Temecula, CA | Full Time

Innovative Medical Group is a growing, physician led medical group with a simple mission: bring exceptional, patient centered primary care to the people who need it most. Along with our post acute work across skilled nursing and long term care facilities in Riverside County, we run a busy primary care clinic in Temecula where we get to know our patients and their families over years, not just visits.

The Opportunity

We’re looking for a Nurse Practitioner or Physician Assistant to join our primary care clinic team. You’ll see your own schedule of adult patients for everything from annual wellness visits and preventive care to chronic disease follow ups and same day sick visits. You’ll have physicians down the hall, a dedicated support team, and the room to build real relationships with your patients.

What You’ll Do
  • Clinic Visits: See scheduled and same day patients for preventive, acute, and chronic care. Perform exams, order and interpret labs and imaging, diagnose, and prescribe.
  • Preventive Care and Wellness: Lead annual physicals, Medicare wellness visits, screenings, and immunizations, and help patients close gaps in care.
  • Chronic Disease Management: Manage patients with diabetes, hypertension, CHF, COPD, kidney disease, and other ongoing conditions using evidence based care plans.
  • Care Coordination: Place and follow up on specialist referrals, coordinate with hospitals and facilities after discharges, and work closely with our physicians, MAs, and front office staff.
  • Patient Communication: Stay on top of your in basket, including results, refill requests, and patient messages, so nothing sits.
  • Documentation and Quality: Complete accurate, timely notes and take part in quality improvement work and performance measures.
Qualifications
  • Education: Master’s degree or higher from an accredited NP program, or completion of an accredited PA program.
  • Certification: Board certified as an FNP, AGPCNP, or PA-C.
  • Licensure: Active, unrestricted California RN license with BRN Nurse Practitioner certification and furnishing number (NP), or active California PA license (PA). Current DEA registration required.
  • Experience: At least 1 year of clinical experience as an NP or PA. Outpatient primary care or family medicine experience strongly required.
  • Clinical Judgment: Confident managing a full patient schedule with a mix of complex and routine visits, and knows when to escalation.
  • Communication: Warm, clear, and patient with patients, families, and the care team.
  • Organization: Self directed, keeps a schedule moving, and closes the loop on follow ups.
  • Commitment to Our Patients: Genuinely cares about delivering respectful, high quality care to every person who walks in.
  • BLS - American Heart Association BLS required

Schedule: Monday - Friday 8 AM - 5 PM (Rotating weekends are required)
