Job summary

Comprehensive Rehab Consultant seeks a clinician to provide specialized PM&R services in a fast-growing post-acute setting in San Diego, CA. You will lead clinical care, collaborate with PCPs, and partner with rehab teams to enhance patient outcomes.

The ideal candidate embraces autonomy, growth, and a commitment to high-quality patient care within a supportive, physician-owned group. A California license and post-acute experience are preferred.

Qualifications

  • Licensed in California as NP/PA or comparable provider.
  • Ability to practice in the U.S. with post-acute focus.
  • Preference for SNF/post-acute experience.
  • Strong patient-centered care mindset.

Responsibilities

  • Provide PM&R/physiatry services as a rehab consultant.
  • Lead CRC representation and collaborate with care teams.
  • Coordinate with PCPs on post-acute conditions.
  • Work with PT/OT/SLP to improve function and life quality.
  • Monitor patient progress and address barriers to therapy.
  • Participate in multidisciplinary meetings and education.

Position Information

San Diego, CA. This is a 40 hour, full-time W-2 role.

Responsibilities:
  • Provide specialized Physical Medicine and Rehabilitation (PM&R)/Physiatry services as a rehab consultant
  • Represent CRC in your facility through strong clinical leadership and collaboration with the interdisciplinary care team.
  • Collaborate with PCP teams to help with the management of conditions common in SNFs such as CVA, progressive neurological conditions, and postoperative orthopedic surgeries
  • Work alongside your PT, OT, and SLP teams to improve patient function and quality of life
  • Follow therapy patients to track their progress in therapy
  • Assess and collaborate on the management of barriers to therapy including joint or neuropathic pain, spasticity, neurogenic bowel, neurogenic bladder, and much more!
  • Become an integral part of the SNF team by attending multidisciplinary meetings, and bridging the gap between therapy and internal medicine
  • Engage in culture building opportunities and ongoing education including monthly Didactic lectures, virtual meetings with your Regional Team, and annual, regional dinners to meet your nearby colleagues
The Ideal CRC Clinician will have:
  • A passion for post-acute healthcare and serving the geriatric population
  • SNF or post-acute experience (preferred)

Qualifications:
  • Nurse Practitioner/APRN/ Physician Assistant
  • California License

  • Experience in post-acute space is preferred

Job Type:Full-time

Pay: Starting at $150,000.00 per year

Schedule
  • 8-hour shift with flexibility, though hours should hover around 9 AM – 4 PM
  • 5-day work week with flexible days (between Monday – Friday); specific days can be selected during onboarding
