Abode Care Partners is seeking a Nurse Practitioner to deliver primary care to seniors and complex patients in their homes across the Springfield, Joplin, and surrounding Missouri areas.

The role emphasizes autonomous work, comprehensive care plans, and collaboration with interdisciplinary teams while supporting ongoing professional growth and a robust benefits package. Weekdays and daytime shifts are typical with mileage reimbursement where applicable.

Qualifications

Active NP license in the state
Current NP National Board Certification
Active state DEA licensure
MSN degree
5+ years nursing experience; 2+ years NP experience preferred
Strong communication skills (verbal & written)
Excellent time management and accountability
Proficiency with medical instruments and equipment
HIPAA and OSHA knowledge
EMR and MS Office proficiency
Valid driver’s license with insured vehicle
Ability to engage in moderate physical activity

Responsibilities

Provide primary care in home settings (skilled nursing, assisted living, independent living)
Conduct clinical assessments and develop personalized care plans
Collaborate with interdisciplinary teams to ensure seamless care transitions
Manage chronic conditions and acute visits with preventative focus
Order and interpret labs and tests as needed

Our NPs provide primary care services to seniors and patients with complex health needs. You will conduct clinical assessments and develop personalized care plans in skilled nursing, assisted living, and independent living communities. You will collaborate with interdisciplinary teams to ensure seamless care transitions and improved patient outcomes as well as manage chronic conditions and acute visits with a focus on preventative health.

Responsibilities

Performs evidence-based medical care by planning, directing, and evaluating patient medical needs
Examines and treats chronic and urgent episodic illnesses, minor injuries, accidents, and other injuries or illnesses
Orders and interprets laboratory and diagnostic testing as needed
Collaborates with caregivers and other healthcare professionals in providing interdisciplinary care and maintaining a comprehensive plan of care
Provides referrals to other healthcare practitioners when necessary
Ensures patient medical records are kept up to date

Qualifications

Active state Nurse Practitioners license
Current Nurse Practitioner National Board Certification
Active state DEA licensure
Master's degree in Nursing (MSN)
Five or more years of nursing experience; at least two years of nurse practitioner experience preferred
Strong interpersonal communication skills - verbal and written
Excellent time management skills with accountability for practitioner's daily workflow
Proficiency in medical instruments and equipment
Strong knowledge base of HIPAA and OSHA regulations
Demonstrated proficiency in computer-based data management programs, electronic medical record (EMR) systems, and Microsoft Office applications
Licensed driver with valid automobile insurance
Ability to engage in moderate physical activity, such as handling objects up to 25 lbs., standing or walking for more than four hours per day, etc.
