Community Care Physicians is seeking a Nurse Practitioner or Physician Assistant to join an established adult primary care practice in Clifton Park Health Park. Work with an experienced physician and two advanced practitioners focused on preventative care and chronic disease management.

The team emphasizes care coordination, accessible communication, and high-quality outcomes, with on-site labs/imaging and a shared EMR across CCP practices. Full-time, Monday–Friday, telemedicine available.

Qualifications

• Current New York State licensure as NP or PA.
• Graduate of an accredited NP or PA program.
• DEA registration or eligibility.
• Strong clinical assessment and decision-making skills.
• Excellent communication and relationship-building abilities.

Responsibilities

• Provide comprehensive primary care services for adults 18+.
• Conduct annual wellness visits, preventive screenings, and routine exams.
• Evaluate, diagnose, and treat acute illnesses and chronic conditions.
• Manage hypertension, diabetes, hyperlipidemia, COPD, obesity, and more.
• Order, interpret, and communicate lab, imaging, and tests.
• Develop individualized plans focused on prevention and disease management.
• Educate patients on medications, lifestyle changes, and goals.
• Collaborate with supervising physician and care team; use telemedicine and patient portal.
• Maintain accurate documentation in the electronic medical record.

Position Overview

Community Care Physicians (CCP) is seeking a Nurse Practitioner or Physician Assistant to join Community Care Internal Medicine Clifton Park, an established adult primary care practice located within the Clifton Park Health Park.

This opportunity offers the ability to work as part of a close-knit team consisting of an experienced Internal Medicine physician and two advanced practitioners dedicated to providing personalized, evidence-based care for adult patients. The practice emphasizes preventative medicine, chronic disease management, care coordination, and building long-term patient relationships. Patients are active participants in their care, supported by a team committed to communication, accessibility, and high‑quality outcomes.

Located in the Clifton Park Health Park, the practice offers convenient access to on‑site laboratory services, medical imaging, women's health services, neurology, weight management, and numerous specialty services, allowing providers to deliver highly coordinated care within one integrated location. All CCP practices share a common electronic health record, promoting seamless communication and continuity of care.

Responsibilities

• Provide comprehensive primary care services for adult patients age 18 and older
• Conduct annual wellness visits, preventive screenings, and routine health maintenance exams
• Evaluate, diagnose, and treat acute illnesses and chronic medical conditions
• Manage patients with hypertension, diabetes, hyperlipidemia, COPD, asthma, obesity, and other chronic diseases
• Order, interpret, and communicate laboratory, diagnostic, and imaging results
• Develop individualized treatment plans focused on prevention, wellness, and disease management
• Educate patients regarding medications, lifestyle modifications, and treatment goals
• Collaborate closely with the supervising physician and interdisciplinary care team to coordinate patient care
• Utilize telemedicine and patient portal technologies to enhance patient access and engagement
• Maintain accurate and timely documentation within the electronic medical record

Qualifications

Required

• Current New York State licensure as a Nurse Practitioner or Physician Assistant
• Graduate of an accredited NP or PA program
• DEA registration or eligibility
• Strong clinical assessment and decision‑making skills
• Excellent communication and relationship‑building abilities

Preferred

• Previous experience in Internal Medicine, Primary Care, Family Medicine, or Urgent Care
• Experience managing chronic disease populations
• New graduates with a strong interest in adult primary care are encouraged to apply
