Job description

Position Overview

Physician‑owned and operated group united by strong values and the mission to provide high‑quality patient care. As an NP/PA PM&R Clinician with Comprehensive Rehab Consultants (CRC), you will work closely with the post‑acute therapy team to optimize rehabilitative outcomes and promote patient independence at a designated skilled nursing facility (SNF) or post‑acute rehab center.

Position Information

Location: Franklin, NH, Concord, NH. Full‑time 24‑hour, W‑2 role.

Responsibilities
  • Provide specialized Physical Medicine and Rehabilitation (PM&R)/Physiatry services as a rehab consultant.
  • Represent CRC in the facility through strong clinical leadership and collaboration with the interdisciplinary care team.
  • Collaborate with PCP teams to help manage conditions common in SNFs such as CVA, progressive neurological conditions, and postoperative orthopedic surgeries.
  • Work alongside PT, OT, and SLP teams to improve patient function and quality of life.
  • Follow therapy patients to track progress.
  • Assess and collaborate on the management of barriers to therapy including joint or neuropathic pain, spasticity, neurogenic bowel, neurogenic bladder, and more.
  • Attend multidisciplinary meetings, bridging the gap between therapy and internal medicine.
  • Engage in culture building opportunities and ongoing education including monthly didactic lectures, virtual meetings with the Regional Team, and annual regional dinners.
Ideal CRC Clinician

  • SNF or post‑acute experience (preferred).
  • Confidence to function autonomously.

Qualifications
  • Nurse Practitioner/APRN/Physician Assistant License.
  • New Hampshire license.
  • Ability to work in the U.S.
  • Experience in the post‑acute space preferred.
  • Positive attitude and desire to deliver best patient care.
Schedule

8‑hour shift with flexibility, though hours should hover around 9AM–4PM.

3‑day work week with flexible days (Monday–Friday); specific days can be selected during onboarding.
