Job summary

Anpex Health is seeking a Nurse Practitioner to provide comprehensive palliative and primary care in a patient-centered setting in Rhode Island. The role emphasizes symptom management, advance care planning, and coordination with an interdisciplinary team.

The NP will conduct histories and exams for patients with complex illnesses, support goals-of-care discussions, and facilitate transitions of care while maintaining comfort and dignity for patients and families.

Qualifications

• Master’s (MSN) or Doctorate (DNP) Nurse Practitioner program completion.
• Experience in primary care, geriatrics, or post-acute care preferred.
• Palliative care training and certification provided.

Responsibilities

• Provide primary and palliative care focusing on symptom management, quality of life, and goals-of-care.
• Lead and document goals-of-care discussions and advance directives.
• Manage pain and symptoms using pharmacologic and non-pharmacologic approaches.
• Collaborate with the interdisciplinary team to review patient status and care plans.
