Job summary

Anpex Health is seeking a Nurse Practitioner specializing in palliative and primary care in New Jersey. You will deliver comprehensive patient-centered care, focusing on symptom management, goals-of-care discussions, and end-of-life planning in collaboration with an interdisciplinary team.

Qualifications

• MSN or DNP as NP program completion.
• Experience in primary care, geriatrics, or post-acute care preferred, but not required.
• Palliative care training and certification provided.

Responsibilities

• Provide primary and palliative care services focused on symptom management and quality of life.
• Lead goals-of-care discussions, including advance care planning and code status.
• Manage pain and non-pain symptoms using pharmacologic and non-pharmacologic approaches.
• Review vitals and labs and escalate when needed to the attending physician.
• Coordinate with IDT weekly to review status and care plans.
• Identify patients for palliative or hospice referral and facilitate transitions of care.
• Educate patients and families on disease trajectory and end-of-life expectations.
• Serve as a clinical resource on palliative care principles for facility staff.
