Location: Methodist Palliative Care

Address: 8303 Dodge Road, Omaha

Work Schedule: Minimum of 2 shifts per month. M-F 8a-4p as needed, rotating weekends and holidays (6-12 weekends per year and 1-2 holidays per year). Rotating on-call from 4p-8a. Casual/PRN employees work on an “as needed” basis to meet business needs. Work days/hours will often vary from week to week.

Responsibilities

Essential Functions

• Provide primary health care for patients by utilizing processes of assessment and diagnosis of health status.
• Obtain patient medical history and data, prioritizing information according to immediate condition or needs.
• Perform physical exams and determine patient mental status.
• Develop and implement treatment, follow-up and evaluation plans for patient outcomes.
• Collaborate with physicians and other disciplines; provide regular and consistent feedback.
• Use appropriate assessment techniques and diagnostic information; perform tests relevant to patient condition and record findings.
• Order appropriate ancillary tests.
• Derive diagnosis from assessment data and diagnostic information as data becomes available.
• Use new or additional data to form a treatment plan.
• Perform telephone triage to determine patient condition and provide appropriate consultation; schedule appointments when needed.
• Incorporate relevant research into clinical practice.
• Provide timely initial patient and family assessments for palliative care consults.

• Implement interventions consistent with the established plan of care in a safe and appropriate manner.
• Apply established therapeutic practices to patient problems.
• Administer medications and immunizations with physician oversight if applicable; prescribe medications according to applicable regulations.
• Generate referrals to appropriate services as needed to further diagnose and progress care.
• Prescribe medications and other non-pharmacologic interventions to promote comfort and symptom management; manage medications and dosages for ongoing treatment of symptoms.
• Pain management (oral, parenteral, IV, IM, subcutaneous, PCA), nausea, constipation, breathlessness, delirium, and other symptoms.
• Appropriately convert medications using equianalgesic/equipotent formulas; manage and preempt common side effects of pharmacologic intervention.
• Recommend and facilitate non-pharmacologic strategies; consult with other team experts as needed (e.g., Chaplains, Behavioral Health).
• Collaborate with palliative care providers as needed, recognizing abilities and limitations.
• Lead and facilitate palliative care consultation meetings and decision making using advanced nursing practice principles.
• Communicate effectively with patients and families regarding diagnosis, prognosis, care, and treatment options.

• Evaluate patient care to determine response to care/interventions for optimal outcomes.
• Assess patient capabilities and limitations related to nursing and medical interventions; address patient and family capabilities and limitations (physical and mental).
• Return patient telephone calls; monitor outcomes and document patient responses to interventions; seek new treatment methods as needed.
• Discuss findings and recommendations for plan of care with attending and consulting physicians and other team members to ensure continuity of care.
• Implement actions and communications that promote continuity of care and optimal transitions within and outside the hospital.
• Collaborate with other disciplines for discharge planning, including financial needs and functional status.

• Provide patient and family teaching specific to health care, including proper use of medications and health/illness prevention.
• Use adult learning principles; check for understanding and provide follow-up questions; connect families with appropriate references and resources.

• Maintain professional behavior and appearance; communicate effectively with physicians, staff, and patients; act as a role model in patient care.
• Participate in meetings, research, and professional organizations; support continuous improvement of services.

• Comply with bylaws, rules and regulations pertaining to Nurse Practitioners or Physician Assistants; follow organization policies and medical protocols; maintain licensure and certifications.

• Maintain complete patient records: document data, treatment plans, interventions, consultations, and billing-related information; complete timely documentation and dictations for new consults/follow-up visits; support reimbursement for services.

• Participate in performance improvement and outcome management; meet with the medical director for chart reviews; identify concerns and participate in quality initiatives and PI data analysis.

• Provide age-appropriate care and education; ensure understanding and comfort with treatment options.

Qualifications

Education

• Graduate of a Master of Science in Nursing (MSN) or Doctorate of Nursing Practice (DNP); nursing license as a Nurse Practitioner required.

Experience

• Minimum of 5 years nursing experience.
• Minimum of 3 years experience in a related clinical area.

License/Certifications

• Current RN license with multi-state compact license or temporary permit while awaiting licensure.
• Current Nurse Practitioner licensure for work state.
• Current American Heart Association or American Red Cross BLS preferred or required within 3 months of hire.

• Strong leadership, problem-solving and medical judgment skills.
• Excellent interpersonal and communication skills with patients, families, and staff.
• Ability to manage multiple tasks simultaneously.

Physical Requirements

• Medium work – exerting up to 50 pounds.
