PeaceHealth is seeking an experienced Nurse Practitioner to join the Inpatient Palliative Care team in Vancouver, Washington. The role focuses on advanced nursing evaluation, clinical assessment, prescribing and monitoring medications, and family teaching as part of the care team.
Ideal candidates have post-graduate experience in inpatient palliative care. APNC certification is highly desired, with strong collaboration across RNs, MSWs, social workers, and case management.
Qualifications
- Must fulfill educational requirements to obtain ARNP license in State of Practice.
- Ability to prescribe medications and manage follow-up care in inpatient setting.
- Experience in advanced nursing practice, preferably in palliative care.
Responsibilities
- Identify and address health promotion and disease prevention needs for the population.
- Collaborate with medical staff; consult, refer, or escalate as needed.
- Prescribe medications and treatments; order labs and diagnostics as appropriate.
- Educate patients and families about care plans and follow-up.
- Maintain accurate medical records and documentation per standards.

- Join a collegial team of two Nurse Practitioners and a Physician Medical Director,seeing "4-5 patients daily, with support from RNs and MSWs.
- 100% inpatient and ability to grow the service line in our community.
- Southwest Medical Center has on-site complex case management teams, social workers, chaplains, and partnerships with the Ray Hickey Hospice House.
- Full-time schedule;no call or weekend requirements.

Within scope of practice and in alignment with both hospital staff bylaws and PHMGs physician supervision expectations, responsible for advanced level of nursing evaluation, clinical assessment, prescribing and monitoring medications and follow-up care for patient. Responsible for patient and family teaching.
- Effectively identifies, evaluates, and addresses disease prevention and health promotion issues of the population in the practice while administering quality patient care.
- Works in an independent and interdependent relationship with members of the medical staff, which allows for consultation, collaboration, or referral.
- Gives directions to nursing and other staff.
- Prescribes medications and treatments and ongoing medication management. Orders lab and diagnostic tests when appropriate and interprets test results.
- Educates patients and/or families about preventive care, medical issues, and use of prescribed medical treatments and/or medications.
- Maintains legible, accurate, and confidential medical records. Documents all medical evaluations, diagnoses, procedures, treatment, outcomes, education, referrals and consultations according to established standards.
- Analyzes new knowledge gained from conferences, workshops, professional literature, or "hands-on training" and assimilates this knowledge into clinical practice.
- Performs other duties as assigned.
- Required : Must fulfill educational requirements to obtain ARNP license in State of Practice
- Required : Nurse Practitioner - Washington or ability to obtain during onboarding process
- Required : Drug Enforcement Admin and
- Required Upon Hire: Basic Life Support and
- Preferred : Advanced Cardiac Life Support
- Demonstrated knowledge of and ability to apply age specific principles of growth and development and life stages to meet each patient’s needs. (Required)
- Effective verbal and written communication skills (Required)
- Interpersonal skills necessary to work productively with patient, families, nursing staff, and other departments. (Required)
- Personal responsibility for ongoing educational growth and collaborating with others to provide solutions for continued patient quality. (Required)
- Experience dealing with terminally ill patients.
- Valid driver's license with an automobile that is insured to state regulations and is in good running order.
- Five years’ advanced nursing practice preferred.
- Advanced Certified Hospice and Palliative Nurse (ACHPN) preferred.
- Time spent on site in medical/hospital setting.
- Vision and hearing required within normal limits (glasses, contacts, hearing aids permitted).
- Ability to communicate and exchange accurate information.
