Qualifications

• Licensed Nurse Practitioner with a valid New Jersey certification.
• Experience working independently and handling your own caseload.

Responsibilities

• Conduct comprehensive patient assessments and manage medical conditions under physician supervision.
• Develop and implement individualized care plans focusing on function and rehabilitation to improve quality of life.
• Collaborate with the healthcare team to ensure comprehensive care, including medication management and therapy recommendations.
• Educate patients and families on treatment plans, health maintenance, and preventive care.
• Monitor patient progress and adjust care plans to achieve optimal outcomes.
