The Program of All-Inclusive Care for the Elderly (PACE) provides comprehensive medical, behavioral, rehabilitative, social, and long-term care services to older adults who meet nursing-home-level care criteria but are able to live safely in the community with appropriate support. The PACE model emphasizes coordinated, interdisciplinary care, allowing providers to develop meaningful relationships with participants while addressing the full spectrum of their healthcare and social needs.
