Qualifications

  • Required MSN from the practicing State Board of Nursing Approved Master's Program.
  • National Board Certification for Nurse Practitioners through an accredited association.
  • Practicing-state licensure as a Nurse Practitioner.
  • Prescriptive Authority from the practicing state.
  • CPR – American Heart Association.
  • DEA Licensure.
  • Criminal Background Clearances (Act 34, Act 33, Act 73, FBI Fingerprinting).

Responsibilities

  • Provides primary healthcare services for non-acute or chronic conditions in primary care settings.
  • Records patient medical history and physical assessment; develops therapeutic care plans.
  • Orders diagnostic tests, interprets results and documents medical records.
  • Communicates with patients, families, and interdisciplinary team; involves supervising physician as needed.
  • Prepares prescription orders and administers medications; wound care.
  • Triage calls and coordinate continuing care; participate in quality improvement initiatives.

Company : Allegheny Health Network Job Description : GENERAL OVERVIEW: This job is an integral part of the care delivery system working together to provide a healing environment, by placing our customers first. The incumbent provides primary healthcare services for non-acute or chronic health conditions in specific populations in accordance with applicable scope and standards of practice and with the policies, values, and mission of the organization within the CRNP scope of practice. Work is performed in a physician office or offices dedicated to Primary Care, weekend, and on-call coverage on a rotational basis, as required.

ESSENTIAL RESPONSIBILITIES

Provides appropriate patient care to include recording patient medical history and physical assessment, developing therapeutic care plan, ordering diagnostic tests, interpreting test results, providing accurate medical record documentation. 25% Communicates and coordinates patient care with patient, family, community health services and interdisciplinary team. Involves attending physician regarding plan of care and involves physician in clinical decision making of any issues that are outside the CRNP's knowledge base or scope of practice. 15% Provides monitoring and continuity of care and coordinates periodic evaluation by physicians as defined in the written agreement between the CRNP and the supervising physician(s).10% Prepares written prescription orders for drugs and controlled substances, administers medication and injections, sutures wounds and provides wound care10% Educates patients and family members regarding health and preventive medicine, medications prescribed, treatment instructions and recommends community resources as appropriate.10% Triages patient telephone calls, provides monitoring and continuity of care10% Participates, develops, and implements performance and/or quality improvement initiatives. Ensures compliance with all regulatory agencies.10% May perform other duties such as manage outpatient concerns and any other duty specific to specialty. 10% Other duties as assigned or requested.

QUALIFICATIONS

Required Master's degree MSN from the practicing State Board of Nursing Approved Master's Program National Board Certification for Nurse Practitioners through an accredited association Practicing-state licensure as a Nurse Practitioner Preferred None
