Primary Care in the MCHS SEMN region is seeking a Nurse Practitioner or Physician Assistant to work as part of a multidisciplinary team to provide care to those living within our community through the Senior Service Skilled Nursing Facility. Within this position the NP/PA will work collaboratively with primary care physicians to provide holistic care including acute, chronic, preventive and palliative care services to patients in nursing homes and skilled nursing facilities. The provision of care includes, but is not limited to, comprehensive evaluation, ordering and interpreting diagnostic tests, prescription of pharmacologic agents and treatments and non-pharmacologic therapies, and counseling patients. Ideal candidates have experience in Palliative Care, Primary care and/or Geriatrics.

$119,870.40 – $167,273.60 per year (at a 1.0 FTE).
