Job summary

Jobtailor is seeking healthcare professionals to perform in-home health evaluations in Maine. You will visit members’ residences, review medications and medical history, perform physical assessments, and conduct diagnostic tests as ordered by health plans.

Travel locally within licensed states and conduct virtual visits as needed. Qualified candidates hold NP/PA credentials, multi-state licenses, and a nursing license with board certification.

Qualifications

• Active, unrestricted nursing license in coverage area.
• Board certification required.
• Multi-state licenses preferred.
• Nurse Practitioner or Physician Assistant qualification.
• Able to cover Penobscot and surrounding area.
• Able to travel locally and within licensed states as needed.
• Able to safely perform duties in members' homes, including environments with household pets, secondhand smoke, and airborne allergens or irritants.

• Provide in-home health evaluations in members' residences


• Conduct approximately 45-minute visits


• Review medications and medical history


• Perform physical evaluations


• Perform additional diagnostic tests when ordered by the member's health plan, including diabetic eye exams, spirometry, and bone density screening


• Use an iPad and clinical workflow for each evaluation


• Build trusted relationships and help improve health outcomes


• Conduct virtual visits as needed


• Provide Signify Health services, including Diagnostic & Preventive Services


• Travel locally and within licensed states as needed


• Help connect members to appropriate care
